answers1: We don't have banned authors or artist in this country. I
mean you may not find their works at the local wal-mart or barnes and
noble but anyone can publish whatever they want. Tons of independent
labels and so forth.

I would suggest you try searching for the name of the book or song on
ebay or the internet and go from there.
answers2: I don't know when exactly they started recording banned
books, but here are some sites:

For bands.... I don't know that many. There are a few metal bands,
here are some songs though: <a
Browneye- There are many books banned from even ENTERING this country,
and if you publish something they don't like they'll have the books
destroyed. Know what you're talking about.
answers3: Chris Crutcher, I think is one.

answers5: Lol you haven't been looking in the right places. However,
banning (at least with fiction) in the US is not a federal matter. It
can be done on a small scale (school librarian chooses not to put a
book in a collection for certain biased reasons) to large scale (a
rare occurence entire state bans the public library access to a book).

The best source is going to be the ALA in association with books
(can't tell you much about music).
They keep track of challenges rather than banning but it should help
you.

In 2005 alone the most challenged authors were Judy Blume, Robert
